Download the revised
SCEIS Access Request Form,
required to request new SCEIS access or changes to a current user's access.
The SCEIS User Access Form has been updated with the following
changes:

Two User titles are now
required: Internal title, such as Assistant Director of Finance,
and HRIS title, such as Financial Analyst III.

Instructions to EMAIL the
form to
SCEISHelp@sceis.sc.gov, rather than faxing it.
To be sure you’re using the
updated form, look for “Rev 2008-03-26” at the top of Page 1.
Processing notes:

FIRST save-as <SCEIS_Access_Request_USERNAME.doc>.
THEN complete the form. Then save again before emailing to
SCEISHelp@sceis.sc.gov.

Signature requests:
Eventually we will change the approval procedure outlined on page 4,
which requests two physical signatures. For now, fill in the information
requested and leave the signature fields blank.
